New Account Proposal Form


Complete the fields below, press the "Create New Account Form" button located at the bottom.

A form that can be printed and signed will be provided once you've pressed the "Create the New Account Form" button. Obtain required signatures. If this proposal is for a new gift account, please forward with appropriate gift documentation to the Central Gifts Office, "Swede" Johnson Bldg., Rm 100. If this is for a new department indirect cost account or patent royalties account, please forward with appropriate documentation to Sponsored Projects Services, University Services Building, Rm. 510. All other new account proposals should be forwarded with attachments (if applicable) to FSO - University Services Building, Rm 502.
