Due to security concerns, our server administrators are no longer allowing us to provide the routable Leave form on E-Forms.
Section A8 of the UA Information Security Policy (http://security.arizona.edu/fileadmin/templates/content/security/pdf/ispolicy.pdf) states "University devices must not provide an active SMTP service that allows unauthorized third parties to relay e-mail messages." The current Leave form does provide the main function of an SMTP relay, which could possibly allow anyone, anywhere to generate fake emails that can pretend to come from anyone and go to anyone.
Although we will not be able to provide a routable form, we have created a Sample Leave Form in Microsoft Word that your department can customize for your needs. This form can be filled out and then saved as a new document that can be sent as an email attachment to your supervisor.
